What is the cost of the Letter of Necessity?  And How long does it stay active/effective for my needs?
Cost:   $15 per LMN | Valid for 1 year

Do I need 2 separate letters if we are under same household/account?
Yes, the LMN is specific to the person with the "medical need"

Do I need a separate letter for exercise/fitness and one for nutrition?
Yes, you need a separate letter for exercise/fitness and one for nutrition if you are purchasing both fitness-related services and nutrition-related services.

Can I get reimbursed for my child?
Yes, if your child has conditions that allow for a LMN.

What's the turnaround time to receiving the letter after the form has been completed and paid for. 
 A maximum of 2 hours but typically in minutes.

What can I get reimbursed for?
Any fitness related activity that qualifies as helping reduce or eliminate your medical condition

Where is the letter coming from?  And Is it Hippa compliant?
The LMN is coming from an digital online medical provider called FLEX.
It is Hippa compliant.   

How do I get a receipt for my monthly fitness dues/PT ?
Send an email to the Cedardale Business Office at [email protected] and let them know you need a receipt for services purchased. Keep in mind you need to obtain your Letter of Medical Necessity prior to purchasing your Cedardale services. So for example, you can not apply to dues already paid but you could apply to dues incurred after you have obtained your Letter of Medical Necessity.

Does it cover the Joining Fee?
Yes the joining fee is covered as it's part of your Membership dues to start a membership at Cedardale

Is it retroactive from the start of my membership?   
No, you can only get reimbursement for related expenses after receipt of the LNM.

Does it cover Tennis/Swim/PB Lessons?
Yes, as those activities are recognized under Fitness related activities.

Can I use HSA/FSA dollars to pay for my membership? How about Training? Nutrition?
If you have a Health Savings Account (HSA) or Flexible Spending Account (FSA) and you have a Letter of Medical Necessity stating you have an existing medical condition that warrants exercise or nutrition services to aid your condition you can use your HSA or FSA dollars.
